This Parent Referral/Resource List Template is tool for parents to help access perinatal mental health services. It includes therapy, medication and crisis services specific to perinatal mental health. The template is designed to be updated and customized by your program/practice. The first page lists helpful tools and resources for professionals as well as instructions for using the template.
